Intern Observes Mentor

  • Intern Observation of the Mentor Teacher” hard copy forms in triplicate will be provided at Intern Orientation.  Additional copies may be printed off the ACP website and copies made if needed.
  • Intern is expected to submit all observations and other forms by due date.  Your university liaison will be giving you either a satisfactory or an unsatisfactory grade for each semester.  The grade is based on several factors.  One factor is turning in work on time.  If you receive an unsatisfactory, you will be required to take an additional semester of internship and will be charged an additional internship fee.
