INSTRUCTIONS

Return the completed application to the department chair/program director by the deadline mentioned on the cover memo. In addition, attach a copy of your latest transcript, a letter of recommendation (see Faculty Member Recommendation For A Student Internship), and a copy of your resume (see page 3 of the application form for details).

The department faculty will review the application, transcript, and reference form. If you meet all requirements, you will be admitted into the internship program and assigned to work with a faculty member on your internship.
