Below is a link to a survey about the course at this point.  The questions are mine, but the web site belongs to another organization.  As far as I know, your answers are completely anonymous and I have no way of finding out who answered questions.  The feedback is for my own use in total quality management and continuous improvement, so your honesty in providing feedback is crucial.  The questions that I have posted will change, and the questions are specific to issues of concern to me at the time.  However, I will always try to provide you with one open ended question to express any concerns, issues, or praise that you think that I should consider in running my course as a service to you.
