New Students (Forms Upload links) 

VA Education Benefits:

New students using VA education benefits, please complete and upload the following documents using your TAMUT Student ID number and the following upload forms:

• DD214, Member 2 or higher version
• Certificate of Eligibility
• Veteran Statement : click form title for the web-base form
• VA Form 22-1995 : For Post 9-11, MGIB AD, and 1606 - this form is required if you used your VA education benefits at another school.
• VA Form 22-5495 : For CH35, DEA - this form is required if you used your VA education benefits at another school.
• NOBE : for 1606 only

Hazlewood Exemption:

New Hazlewood Exemption students, please complete and upload the following documents using your TAMUT Student ID number and the following upload forms:

• Veteran Students:
  • HZ Initial Application : click form title for the web-base form
  • DD214, Member 2 or higher version
  • Certificate of Eligibility 
  • TVC Hours Log
• Legacy Students:  Veteran is passing unused hours to their dependent children.  Veteran Documents above plus:
  • Birth Certificate
  • Marriage License - if dependent child is a step child.
  • IRS 1040 - if sponsorship is by claiming on taxes
  • TVC Hours Log - initial term, both Veteran sponsor and child must submit an Hours Log from their individual TVC account
• Dependent Spouse / Child:  Veteran is deceased as a result of a service related injury or illness, or Veteran is 100% total and permanent disabled or meets the eligibility requirements for individual unemployability per VA guidelines. Veteran Documents above plus:
  • Birth Certificate (for dependent child)
  • Marriage Licensed (for spouse and/or if child is a step child)
  • IRS 1040 - if sponsorship is by claiming on taxes
  • Veteran's VA Disability Rating Letter
