Accident Reporting

All employees are obligated to report any accident or injury immediately.  Failure to report in a timely manner may cause complications in potential Workers Compensation coverage as well as lead to inability to properly investigate the accident to correct any causes.  

For safety reasons, all accidents involving students, visitors, or employees should be reported immediately to UPD at 6611, regardless of severity.  Medical emergency matters will be handled by UPD.


Employees must report all accidents to their supervisor who will complete the Incident Report .  EHS can assist in the entry of the report into Origami if needed.   Origami help can also be found here.  Select 'Injured employee', as well as any others that apply, as the type of incident.  

Students, visitors, and all other non TAMUT employees should also report all accidents to UPD immediately.  The accident should be documented using the Incident Report.  

University owned, leased, or rented vehicles or mobile equipment accident or theft also require an employee to complete a Incident Report
