Incident Reporting
All accidents and injuries involving students, visitors, faculty and staff should be reported immediately to their direct supervisor.
All emergency matter requesting immediate assistance will be handled by UPD. ALL accidents, injuries and safety concerns need to be entered into the campus incident reporting system, Origami.
Employer's First Report of Injury or Illness

Incidents involving employee injury involving contaminated sharps must ALSO be reported to the State of Texas Department of State Health Services. The form to be used can be found here. Please forward to the address listed and forward a copy to ehs@tamut.edu .
